Over the past 20 years, Klebsiella pneumoniae become the leading cause of pyogenic liver abscess in many Asian countries including Korea. And diabetes mellitus is known risk factor of Klebsiella Pneumoniae liver abscess, and it appears to be a significant risk factor for systemic metastatic complications, especially such as endogenous endophthalmitis. A 40-year-old female was admitted for diabetic ketoacidosis which was precipitated by Klebsiella Pneumoniae liver abscess. She was not diagnosed diabetes before and her clinical manifestation showed septic pneumonia, renal abscess and catastrophic endogenous endopthalmitis also. Only three cases of diabetic ketoacidosis precipitated by Klebsiella Pneumoniae liver abscess were reported in known diabetes patients in Korea. Here we report on clinical features of diabetic ketoacidosis which was developed in patients without known diabetes history and accompanied with catastrophic endopthalmitis as the first case of Korea.
